Attachment 1, Glacier Peak Mining Operation <br />Forest Service Evaluation of Plan of Operations <br />A: Required Changes/modifications/special mitigations for plan of operations <br />On the Smokey Hawk, Wolf Fang, Elk Horn, Blue Smokey and Qui-Buc 2 unpatented <br />mining claims, Glacier Peak Mining, aka Joe Dorris, is authorized to use mechanized <br />equipment and hand tools to conduct exploration and mining activities and to have up to <br />one-third of an acre of total disturbance on these unpatented claims at any one time <br />during the authorized period. <br />No road construction is authorized. All vehicle travel will be over existing roads. No <br />motorized travel off existing roads is authorized. No road maintenance is authorized <br />without approval of the Forest Service authorized officer. Please contact Jeff Hyatt if <br />you feel the roads you aze using need to be maintained. <br />All waste will be removed from the National Forest for disposal in an approved facility in <br />accordance with state and local regulations. <br />No chemicals or additives may be used in water used for washing minerals on site. <br />Please contact Jeff Hyatt before removing trees lazger than 8" in diameter. If trees are <br />removed, limbs will be cut from the tree bole, then lopped and scattered. Slash will be no <br />deeper than 12 inches on the Bound. Bole material larger than 4 inches diuneter will be <br />removed to location where the public can access it for firewood. Bole material does not <br />have to be bucked into short pieces. In lieu of cutting, trees may be pushed or excavated <br />with mechanized equipment, with prior Forest Service approval. All resulting stumps <br />will be buried in the adjacent pit or trench when reclamation occurs. A firewood permit <br />or other forest products permit is required to remove any trees not necessary for mining <br />operations. <br />Individual open pits and/or trenches will be fenced or flagged with caution tape, chicken <br />wire, snow fence or other material approved by the Forest Service, when the pits are open <br />and the operator, his field representatives or employees/guests are not on site to provide <br />for public safety. <br />The operator will comply with all federal, state and county regulations and requirements <br />for occupancy, use and mining on National Forest lands, and will obtain all permits or <br />licenses required by any federal, state or county agency. <br />Soils. Water Quality and Riparian: <br />Best management practices will include the following specific requirements from the <br />Forest Service Watershed Conservation Practices Handbook, FSH 2509.25: <br />On slopes greater than 20%, the operator will install and maintain containment structures <br />on the downhill side of all disturbed areas until vegetation is established at 50% or <br />
