See 'exhibit B for gravel pit location. <br />2.. This permit is issued in accordance with the State Highway Access <br />Code, and is based upon the information submitted by the nermittee. <br />This permit is only for use and puroese stated in the application and <br />permit. Any changes ir, traffic volumes, drainage, type of traffic or <br />f other operational aspects may .ender this Fermit void, requiring a <br />new Application fer Access Permit to be submitted For review by this <br />office, based upon existing and anticipated Fu cure conditions. <br />3. (Fully executed means, a Colorado Department <br />of Transportation (COOT) Access Permit number assigned to the permit, <br />stamped "PAID', and signed and dated by both the Permittee and an <br />I- authorized representative of COOT.) <br />5. Prior to starting construction, the contractor must submit a traffic <br />I- control plan (according to the latest MUTCD) to Mr. John Schwartz, <br />I Senior Highway Maintenance Foreman, in Pueblo. He can be contacted <br />by telephone at (719) 596-5927. If any traffic control devices are <br />r evident within 50 feet of the constmction area, the contractor must <br />I contact the District Traffic Engineer in Fuebl o, Mr. Terry Shippy, at <br />(719) 596-5992. <br />I 6. Coordinate construction work with Mr. Schwartz. <br />7. Minor Eie1d changes may be required by the Senior Nighway Maintenance <br />1~ Supervisor. Any changes to the plans by the contractor must be <br />approved by the Senior Highway Maintenance Supervisor. <br />8. Any additional permits required by other Federal, State, local <br />government agency or ditch company are the responsibility of the <br />applicant and property owner. <br />I 9. Contractor shall follow the applicable construction specifications <br />set forth by the Department of Highways in the latest manual of <br />'Standard Specifications for Rca3 and Bridoe Construction'. <br />~,,; 10, Work shall UEGIN AFTER H;30 a.m., and all equipment shall be off the <br />roadway BEFORE 4:30 p.m, each day. No work is allowed within the <br />~ hightuay right of way on weekends or holidays. Dlo construction or <br />~~ private vehicles may be parked on highway right of way. No <br />construction materials may be stockpiled on highway right of way. <br />~~ 11. Permittee i9 responsible for any utilities and/or traffic control <br />~• devices disrupted by the construction of this driveway, and all <br />expenses incurred for repair. There are existing utilities on the <br />Fighmay rigbt of way by permit. Owners of thosa utilities must be <br />ccntacted. Any work necessary [e protect e..is ling permitted <br />utilities, such as encasements, bulwarks, etc., will be the <br />responsibility of the Permittee. <br />12. Any damage to any existing highway Facilities shall be repaired prior <br />to continuing other work. <br />• 13, The access shall be constructed 35 feeC iri width, with 3D foot radii, <br />and at a 90° angle to the highway. See Exhibit C for access <br />construction. <br />19.
