i~ <br />Cripple Creek & Victor Gold Mining Company November 3, 2004 <br />Mr. Ron Roberts -2- 043-2170 <br />' Construction of the modified Amendment No. 7 boundary area required exposing the existing <br />Phase II and III liner anchor trenches, and excavation of approximately 770 linear feet of new anchor <br />trench along the western perimeter. Golder observed and documented the deployment, seaming, <br />' defect repair, and testing (Destructive and Non-Destructive) of approximately 75,565 squaze feet of <br />80-mil single-sided textured LLDPE geomembrane deployed by Poly-flex. The as-built <br />geomembrane panel layout is presented in Figure 1. Geomembrane was supplied and tested in <br />' accordance with the Phase N VLF Construction Specifications and was placed to complete the <br />composite liner system. CQA observation records and test results for the installation of <br />geomembrane are presented in Attachment B. <br />' Golder visually monitored the placement of approximately 5,100 cubic yards of DCF over the <br />geomembrane associated with the modified Amendment No. 7 boundary. Golder observed that DCF <br />was placed to maintain a thickness of 2 feet of DCF over the geomembrane. <br />Other information required by the Phase IV VLF Construction Specifications for the placement of <br />SLF and geomembrane, including soil surface acceptance certification, geomembrane certification <br />summazies, equipment calibration certificates, and resumes of Poly-flex personnel, aze presented in <br />Attachment D. <br />The modified Amendment No. 7 boundary area was constructed in accordance with the Phase IV <br />VLF Construction Specifications. <br />Golder appreciates the ongoing opportunity to provide engineering and CQA services for the Cresson <br />Project.
