Initial approval for the <br /> construction of the VLF in this azea occurred under Amendment No. 7 to the Colorado Division of <br />' Minerals & Geology (DMG) permit number M-1980-244. <br /> The modified Amendment No. 7 boundary liner encompasses an area of approximately 68,700 square <br />' feet (based on the gading plan) at the contact of the existing Phases II and III Valley Leach Facility <br />(VLF) <br /> . <br />Construction activities were performed by Ames Construction Inc. (Ames) and Poly-flex <br />' Construction Inc. (Poly-flex) during the period of September 5 through September 25, 2004. Golder <br />observed and documented the construction activities including subgade prepazation, placement of <br />Structural Fill {SF), Soil Liner Fill (SLF) placement, anchor trench excavation, geomembrane <br />' deployment, and Drain Cover Fill (DCF) placement. <br />Golder conducted laboratory testing as required by the Phase N VLF Construction Specifications for <br />t DCF, SF, and SLF, Tables 1 and 2 present a summary of the Technical Specifications and testing <br />frequency. The laboratory test results are summazized in Tables 3 through 5 and aze also presented in <br />Attachment A. Attachment B presents geomembrane observations and test results (including ambient <br />' temperatures). Attachment C presents weekly construction reports. Attachment D presents subgrade <br />acceptance forms, geomembrane and resin certiFications, equipment calibrations, Golder staff schedule, <br />and Poly-Flex resumes. <br />' Field material testing was conducted by Golder on SLF placed in the modified Amendment No. 7 <br />area. The field testing of approximately 2,500 cubic yards of SLF included monitoring of placement <br />and preparation for geomembrane deployment, testing in-place moisture and density, and performing <br />' depth verification testing. Soil Liner Fill placement was performed in accordance with the Phase N <br />VLF Construction Specifications. Results and locations of field density and depth verification testing <br />aze presented in Tables 6 and 7, respectively.
