Following are a few <br />items that need to be addressed in order to comply with the minimum requirements of the <br />Act/Rules and Regulations: <br />1. Application - OK <br />2. Exhibit A - OK for existing permit. Does not describe the proposed new acreage in <br />Section 29. Please revise this exhibit to include the expanded acreage in Section 29. <br />3. Exhibit B -Does not show the location of the exiting permit azea nor the proposed <br />expanded acreage. Please revise this exhibit to show the location of the entire permit area <br />(existing and new areas). <br />4. Exhibit C -The maps are lacking several items: a) the maps aze not signed by the qualified <br />person who prepared them, b) the maps do not contain the exhibit letter or the type of map <br />(eg. pre-mining, mining, etc.), c) the pre-mining map does not show the names of the <br />adjoining landowners. Are there landowners within 200 feet of the affected land boundary <br />other than Playfair Land Company? If so, you need to show the names on the pre-mining <br />map. If the neazby land ownership has not changed since submitting the 1993 Conversion <br />application then specify where the information can be found, d) the maps do not contain <br />the name of the permittee (operator), e) an attempt was made to show the existing <br />topography however, the projected topographic elevations in the southeast comer of the <br />permit area show that the stream, which flows south, travels from an elevation of 9,980' to <br />an elevation of 10,030'. Also, the topographic lines indicate that you aze mining a steep <br />mountainside which, of course, is not the case, f) indicate on each map that the survey <br />lines represent the permit boundary, g) delineate the existing disturbance on the pre- <br />mining and mining plan maps, h) show the approximate location(s) of the topsoil pile(s) on <br />the mining plan map, i) on the pre-mining map, describe the type of present vegetation <br />covering the affected lands or provide a narrative where the information can be found in <br />the existing permit.
