Dear Mr. Pittington.:
On September 3, 2004, the Division of Minerals and Geology determined that the 110 to 112 Construction Materials
Reclamation Permit Conversion Application package for the Russell Gravel Pit, Permit No. M-1994-096 is incomplete.
Preliminary review of the ird'ormation received determined that two (2) copies of the following items most be received before the
Division can consider your application as being submitted and technical review can begin:
Mr. James Krakel did not submit an amendment application to the Division to change the Mine Plan and the
Reclamation Plan.

(2) All fee, warranty and processing requirements shall apply as though the Conversion application were a new
permit application. A fee, as specifies in Section 34-32.5-125(1)(a), C.RS., shall be submitted at the time of
application submittal.
(3) Contents of application:
(a) except as otherwise indicated in this Subsection 1.11.2, the Operator shall provide all the information
required by the Act and these Rules for the size of operation. However, the Operator need not supply
any i~'ormation required by the provisions of the Act which has been previously supplied unless such
information is different from that in the original application. However, the Applicant shall clearly
describe where in the original application and supporting documents the information not included in
the conversion applicatioq but necessary to render the conversion appliption technically adequate,
may be found.
(b) In additioq the application shall show:
(i) the area mined or disturbed; and
(ii) the area reclaimed since the original permit application.
