4. See last item of no. 3 above. <br />5. Please clearly identify the permit boundary on the map. Please be aware that if the edge of the <br />asphalt is delineated as the permit boundary, any disturbance off the edge of the road may be <br />considered as off-site impacts. It is recommended that the permit boundary be wider than the <br />road, though the acreage specified in this application cannot be increased without new notices <br />and starting all applicable timeframes anew. <br />6. Because this map serves as both Mine Plan and Reclamation Plan Map for this portion of the <br />permit area, it should be labeled Exhibit C and Exhibit F with a note specifying that the road is <br />permanent. <br />Exhibit D -Mine Plan <br />7. Please provide specifications for road construction including, length, width, base construction, <br />asphalt thickness, any ditches and culverts. <br />Exhibit E- Reclamation Plan <br />8. Please be aware that the asphalt road must be in good condition to be eligible for final release. <br />Please commit to maintain the asphalt surface in good condition, for the life of mine, or costs will <br />be included in the reclamation estimate for resurfacing the road. <br />Exhibit L -Reclamation Costs <br />9. See No. 8 above. <br />Exhibit S -Permanent Man-made Structures <br />10. Several structures on the application map, on or within 200 feet of the proposed amended permit <br />area (assumed since no permit boundary is indicated) are not addressed in this exhibit. Please <br />address these structures, and any permanent man-made structures in this area that were left off <br />of the map, as required by Rule 6.4.19.
