Phase IVb weeldv Summary ?vo. 22 Pale 3 003-2203 <br />Phase Nc <br />Ames performed no construction activities in the Phase IVc area during this reporting period. <br />Field Monitoring and Testins Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed the placement of secondary and tertiary HVSCS piping in the Phase IVb PSSA and <br />Phase IVb OSA, verifying constmction per the project plans. <br />Golder visually observed the placement of DCF in the Phase IVb PSSA and OSA azeas. Golder <br />CQA monitors (one monitor per dozer when dozers are greater than 50 8 apart) observed DCF <br />placement and verified minimum thickness of 2 feet for finish grade, 4 feet for FIVSCS primary pipe <br />locations, 5 feet for 773 equipment, and 10 feet for haul road construction and equipment. <br />Golder visually observed removal of DCF from the Phase III/IVb tie-in area. Damage caused to <br />geomembrane was marked for subsequent repair. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase N6 OSA <br />for subgrade preparation, thickness, compaction, moisture content, and materials. The Golder CQA <br />Ea-hworks monitor performed nine nuclear moisture-density tests and performed 4 nuclear <br />moisture-density retests on SLF that required reworking (SLNb-147a through SLNb-li0a) located <br />within the Phase IVb OSA. The retested SLF met the requirements of the specii cations. Field test <br />results are presented in Attachment B. <br />Golder observed geomembraae deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />de...,..~cti; e). Laboratory index <br />testing was conducted on samples of Soil Liner Fill (SLFb-12, SLFo-i4), Select Drain Cover Fill <br />(SDCFb-Ol), and Drain Cover Fill (DCFb-06 through DCFb-08) obtained dosing this and the <br />previous reporting period to verity suitability of materials used. Laboratory testing of SLF sample <br />SLFb-13 obtained during the previous reporting period is pending.
