Attached is a letter from the
Division to Holnam, Inc. regazding the conditions placed on approval of AM-O1 during the April 28, 1994 Mined Land
Reclamation Boazd Hearing.
The Division could find no evidence in our files that any of these stipulations have been comp]ied with by Holnam, Inc.
or Holcim (LTS), Inc. Please review the List of stipulations, and inform the Division as to their status. To avoid potential
future enforcement actions for non-compliance with State Statutes, Rules and Regulations, and/or conditions specified
in your permit for this site, documentation of compliance for each stipulation must be submitted to our off ce by July
12,2004.
If you have any questions, please contact me at (303) 866-4943.
Sincerely,.
/~~~~
Enca S.
