Please obtain and submit to DMG a letter authorizing Meurer & Associates to act as <br />DMG's direct permitting contact on behalf of Mr. Mike Siegrist until the conclusion of <br />this amendment process. <br />Rule 6.4.5 Exhibit E- Reclamation Plan <br />7. The Division received comments regazding this amendment from the Office of the <br />State Engineer and the Corps of Engineers on Apri] 6, 2006. These comment letters <br />were provided to the operator during the pre-operation inspection of April 11, 2006 <br />(and aze enclosed with this adequacy letter, as well). Additionally, the Division <br />received a comment letter from the Colorado Historical Society on April 13, 2006, <br />which is also enclosed. Please address these letters accordingly. <br />8. Please further describe the dimensions of the topsoil pile(s) that will be stored on site <br />and state the anticipated amount of cubic yards of topsoil that will be stored on-site at <br />all times. <br />9. Please provide additional information regazding revegetation techniques that will be <br />implemented on site - i.e. mulching (application method and rate/acre) and fertilizer <br />(application method and rate/acre). <br />10. Please provide a vegetation description per Rule 6.4.3(e) -this information can be <br />obtained from Natural Resources Conservation Service. <br />11. Please provide additional information regarding the location and source of borrow <br />material to be used on site. If material is imported from off-site, please refer to Rule <br />3.1.5(9) (copy enclosed) and supply the information required by this rule. <br />12. Please provide an explanation as to the use and/or disposal of the concrete stockpiles <br />found on site during the inspection. <br />13. Please describe how the asphalt road at the south edge of the ponds (between the main <br />pond and the St. Vrain) will be reclaimed. <br />14. Please provide documentation revising the grade of the swimming area from 3h:ly to <br />Sh:ly as required by Rule 3.1.5(7). <br />Rule 6.4.6 Exhibit F- Reclamation Plan Map <br />15. Please provide the acreage of the permitted area and clearly illustrate the size and <br />location of the temporary sediment ponds on a map. <br />16.
