The Division of Minerals and Geology (Division) is required to make an <br />approval or denial decision no later than June 27, 2006, therefore, your response to the following <br />adequacy review concerns should be submitted to the Division as soon as possible. <br />1. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, pmfessiona] <br />engineer, or other qualified person. The maps submitted to the Division were not <br />signed. Please resubmit a signed copy of the maps. <br />2. As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public <br />notice circulated in the local newspaper, all owners of record of surface and mineral <br />lands, easement holders, and owners of record of surface lands that aze within 200 feet <br />of the boundary of the affected land. Proof of notice may be return receipts of a <br />Certified Mailing or by proof of personal service. <br />3. Please submit the following information for all permanent structures within 200 feet of <br />the affected azea, including all utilities and easement holders: <br />a) List of owners names and addresses <br />b) Map cleazly associating the owners with the location of these structures <br />Rule 6.4.4 Exhibit D- Mining Plan <br />4. Please designate if disturbances such as mine benches or water impoundments will <br />occur, and if so specify their dimensions and locate on a map. <br />5.
