Walther <br />Executive Director <br />Ronald W. Cattany <br />Division Director <br />The Division completed a technical review of your Amendment Application. Following are a <br />few items that need to be addressed in order to meet the minimum requirements of the <br />Act/Construction Rules and Regulations. <br />Pursuant to Rule 1.10, an amendment application to a permit shall be submitted with the same <br />content as required for an original application, except that the Applicant will not be required to <br />submit any information which duplicates applicable previous submittals. However, the Applicant <br />shall clearly describe where in the original application and supporting documents the information <br />not included in the amendment application, but necessary to render the amendment technically <br />adequate, may be found. If any of the below noted items have already been addressed in the <br />original permit application, or any subsequent permit revision, please clearly describe where the <br />information can be found. <br />Only those items that need to be addressed are listed: <br />Exhibit C -the Mining Plan Map is not signed pursuant to requirements of Rule <br />6.2.1(2)(b). Please have the qualified person who prepazed the map sign the map. <br />The Exhibit C map must show the type of vegetation covering the affected lands (Rule <br />6.4.3(e)). <br />Does the map boundary identify and outline the permit boundazy or the affected land <br />boundary (or are the boundaries the same)? Pursuant to Rule 6.4.3, the total azea to be <br />mined and the area of affected lands must be identified on the Mining Plan Map. <br />2. Exhibit D - a description of the size and location of each azea to be worked during each <br />phase should be included in this Exhibit (Rule 6.4.4(e}(ii)). <br />The Mining Plan should outline the sequence in which each stage or phase of the <br />operation will be carried out (Rule 6.4.4(e)(iii)). <br />Describe the major components of the mining operation such as: roads and access routes, <br />pit, office, shop/maintenance buildings, plant, and processing facilities. <br />
