Phase IVb Weekly Summary No. 9 Page 2 003-2203
• On January 20`", a Caterpillar D9 wide pad dozer damaged the lower 100 mil single sided textured
geomembrane in the Phase IV PSSA near panel 354, creating puncture and scratch penetration
damage to the geomembrane. Ames used a Caterpillar 330 excavator on January 20`h and 215` to
remove LVSCF from the area where the geomembrane damage occurred. As Polyflex was not
onsite, CC&V performed temporary geomembrane repairs on these areas on January 23`d, 2003. The
temporazy repairs were surveyed and will not be covered with LVSCF until Polyflex returns to
perform certifiable repairs and tests on the damaged geomembrane area.
Ames performed SSF backfill on underground working UG-315, and performed SF backfill above the
previously placed geogrid at UG-316.
Phase IVc
Ames placed a Concrete Plug (CP) at UG-304. Ames placed Cemented Rockfill (CR) at UG-448, UG-
504 and UG-269. Ames placed geogrid at UG-504. Ames placed Structural Fill (SF) over previously
remediated workings at UG-234a, UG-236, UG-498, and UG-504. Ames performed excavations at
workings UG-304, UG-504, UG-269, and UG-274 in preparation of geogrid placement.
Ames performed local cut to fill of Structural Fill (SF) material down slope to the temporary stockpiles
above and below the old Connolly haul road elevation.
Field Monitoring and Testing Activities
. Golder observed construction activities associated with Phase IV earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications. Removal of oversized and deleterious material from all fill
aeeas was observed. Golder observed Structural Fill (SF) placement in Phase IV for materials, lifr
thickness, moisture content, and compactive effort. Golder obtained samples of SF for laboratory
index testing. Materials supplied met project specifications.
Golder observed placement of SF, geogrid, CP and CR for underground working remediation. These
materials were placed in accordance with the drawings and specifications into excavated or open
underground workings. CP and CR was observed full-time for correct aggregate size, QC observation,
cement ratio calculations, and placement and consolidation monitoring.
Golder provided full-time observation of LVSCF placement for correct material, road thickness,
placement thickness, drop height, taper, and liner damage. LVSCF was placed in accordance with
the specifications. Golder obtained samples of LVSCF for laboratory index testing. Materials
supplied met project specifications.
Laboratorv Tests
Golder obtained four samples of LVSCF (LVSCFb-09, 10, 11, and 12) for laboratory testing.
Golder obtained one sample of Structural Fill in the Phase IVc area (SFc-OS) for laboratory index
testing. The samples tested met the requirements of the specifications and results aze presented in
Attachment B.
Phase1V609 Jan19-Fe601, ]OW Golder Associates lnc.
