The decision date for your application is scheduled for March 7, 2005. <br />Therefore, [would appreciate a response from you on or before February 25, 2005, (Please be sure to <br />include two (2) copies of your response). Please be advised that your conversion application maybe <br />deemed inadequate and may be denied on March 7, 2005 unless the following adequacy review items are <br />addressed to the satisfaction of the Division. <br />--. <br />,6:4:2 EXHIBIT B - Indez Map <br />1. In conformance with Rule 6.4.2, please provide a revised copy of the Index Map which shows the <br />direction and distance to the nearest town, labeled County Roads used to access [he Riley Pit, and location <br />of the Riley Pit (appears to be dark shaded area). <br />F6:4.3 EXHIBIT C -Pre-Minine and Minine Plan Map(s) of Affected Lands <br />2. In accordance with Rule 6.2.1)2), please revise all maps (except for the Index Map) to conform to the <br />following criteria: <br />show name of the Applicant <br />must be prepared and signed by a registered land surveyor, professional engineer, or other <br />qualified person <br />give date prepared <br />at a map scale of not larger than 1 inch = 50' nor smaller than 1 inch = 660' <br />include map scale, appropriate legend, map title, date, and north arrow <br />3.
