5) Exhibit D - provide a statement regarding the disposition of the buried septic sludge deposits on the county ]ands. Will any of the deposits be included in the permit area? If so, the location of the deposits must be easily identifiable in the field and must be shown on the Exhibit maps. Describe the method of mining in and around the overhead power line and underground gas line. Provide a copy of all setback limits or depth limitations from the structure owners (if any). Describe, in detail, how the structures will be protected from possible damage during mining and reclamation operations. 6) Exhibit E - provide a list detailing the percentage of each of the proposed grass species. Importation of backfill material must be reported to the DMG pursuant to the provisions of Rule 3.1.5(9). This includes the material that was recently dumped in the south end of the existing permit..- 7) Exhibit F -the Reclamation Plan Map must comply with all applicable requirements of Rule 6.2 (see Exhibit C above). 8) Exhibit H -Wildlife Statement -the wildlife statement in the original permit should still apply so, the applicant should provide a statement that the original wildlife statement can ' be found on page 8 of the original permit. 9) Exhibit:M - is the applicant zequired to obtain a county;conditional use permit or a special use permit? 10) Exhibit S -the applicant must comply with this Rule regarding the overhead power line and the underground gas line. Please provide a copy of the most recent proof of publication and other required proof of notice (if available). A copy of this adequacy letter, and the applicant's response, shall be filed with the Clerk and Recorder and the applicant shall submit proof to the DMG that the Clerk and Recorder received the documents.
