u
•
•
Phase IVb Weekly Summary No. 20 Page 3
Field Monitorine and Testing Activities
003-2203
Golder observed construction activities associated with Phase fV earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications.
Golder observed the welding and pipe fitting of the HVSCS steel riser piping and elbows out of the
sump and onto the floor of the upper geomembrane of the Phase IVb PSSA. Golder verified
protection of the underlying upper geomembrane prior to final placement of the steel pipes. Golder
observed the placement of the connecting 26-inch perforated HVSCS equalizer piping between the
welded piping as shown in the project plans. Golder also observed the placement of secondary and
tertiazy HVSCS piping, verifying construction per the project plans.
Golder observed construction of the Leak Detection Trench in Phase IVb for trench dimension (1'
wide by 1' deep), slope (minimum 1%), materials, geomembrane and pipe placement, and geotextile
overlap.
Golder visually observed the placement of DCF in the Phase IVb PSSA area. Golder CQA monitors
(one monitor per dozer when dozers aze greater than 50 R apart) observed DCF placement and
verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations, 5
feet for 773 equipment, and 10 feet for haul road construction and equipment.
Golder visually observed the placement of SDCF in the vicinity of the Phase IVb vertical risers on a
part-time basis.
Golder observed construction and placement of BF in the Phase IVb OSA area. Golder CQA
monitors verified minimum thickness of 1 foot for finished grade as required per the project plans.
Golder observed anchor trench excavation in the Phase IVb PSSA and OSA for proper location and
dimension (2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the
specifications.
Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA
for subgrade preparation, thickness, compaction, moisture content, and materials. Golder CQA
monitors performed seven nuclear moisture-density tests (SLNb-122 through SLNb-128), and eight
depth verification checks (SLDb-78 through SLDb-85) on SLF placed within the Phase IVb OSA.
The tests performed met the requirements of the specifications with no additional re-testing, and the
nuclear moisture-density results summary is presented in Attachment B.
Golder observed geomembrane deployment, documenting ambient air temperature, panel
dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and
destructive). Geomembrane placed in the Phase IVb OSA met the project specifications.
anew zo m~ei-m~a zooa.aa Golder Associates lnc.
