We started the inspection on <br />3/30/93 and finished on 4/2/93. The site conditions were generally snow covered. <br />Tha areas that did „ct have a snow cover were the active areas. <br />Mr. Beverlin accompanied us in the field on 3/30 only, and Mr. Routten <br />accompanied us from 3/31 to 4/2. The mine was inactive on the John L. Lewis <br />holiday, 4/1. <br />The DMG permit term is from 11/16/91 to 11/16/96. The CDFS permit, #CO- <br />0032638, expired on 3/31/92, but the company has been given an extension. <br />There are five identified discharge points in this permit. 003 and 006 have a winter <br />variance because of inaccessibility. The pond inspections have been conducted as <br />required. All ponds have a construction certification. The blasting notice was last <br />published 5/21/92. There are four SAE's approved for the mine but field conditions <br />indicate that the drainage from these locations would go to a pond anyway. All <br />haul roads have certifications; the MoffaT road is the only completely post-law road. <br />The liability insurance, HDCG1 501778-8, is with Insurance Co. of Amerca and <br />expires 3/1!94. P&M has identified "operations" throughout the country and each <br />operation is covered in the policy. An operation may have more than one permit <br />number though. P&M has two bonds in effect for the Edna Mine for a total of <br />$7,121,176. The bonds, #'s U80 61 57-27 and U80 61 57-28, are with Bond <br />Reliance Insurance Co. <br />The permit is a confusing mixture of pre-law, interim, and permanent program <br />Lands. Some pre-law lands have a bond release under the old laws. The permit <br />boundary is broken up by these various jurisdictional areas. <br />
