On April 21, 2000 the Division of Minerals and Geology issued a conditional approval of the technical
revision application submitted to the Division on April 12, 2000 addressing the following:
Grouting a portion of the abandoned No. 2-Dam decant system and the installation of parallel
gravity lines to route No. 3-Dam seepage to the No. 1-Dam seepwater collection system.
The tetras of the technical revision TR-09 approved by the Division aze hereby incorporated into Permit
No. M-77-342. The conditions of the Division approval are listed below. Please note that all other
provisions of Permit No. M-77-342 remain in full force and effect.

Stipulation No. Description
The 3-Dam seepage pipelines will no[ be put into service until the Division has
issued written acceptance of the certification of the facility as required under
Rule 7.3.2(2) of the Hazd Rock/Metal Mining Rules and Regulations. The
certification shall be accompanied by a construction report that will detail the
installation of the pipelines, including pipe and valve specifications, adescription
of the inlet structures, seepage barriers and south dike reconstruction, a
description of any problems that occurzed and their resolution, and a description
and test results of pipeline integrity testing.
The 3-Dam seepage pipelines will be tested for integrity at least once per year
and a description of the testing and results will be provided in the annual
reclamation report for the Henderson Mine and Mill
Within 60 days of completion, a grouting report will be provided for the 2-Dam
decant system grouting operation. The report will detail the installation of the
