Listed below are remaining adequacy review comments on the above amendment application,
that will need to be resolved prior to Division approval. These review comments are remaining
from the previous July 5, 2006 " 2"d Adequacy Review Comments" memo to Deb Gonima and
Carl Mount.
6.4.5 EXHIBIT E -Reclamation Plan
2. The applicant has committed to providing the Division with a copy of the QaQc Specifications
to be implemented during slurry wall construction prior to beginning construction of the slurry
wall. As part of this commitment, the applicant will need to provide the Division with a copy of
the specifications at least 30 days prior to slurry wall installation and provide the Division with
written verification of the date when slurry wall installation began.
3. Please address the following concerns with respect to the Groundwater Monitoring Plan
recommended by the Applegate Group.
b. Revised Map -The applicant has provided a copy of the Tab 14 Map ("Lake Reclamation
Exhibit") which shows the locations of three (3) proposed groundwater monitoring stations near
the western portion of the project. The map will need to be revised to include the location(s)
where mitigation measures such as perimeter drains will be installed upon a finding of a
groundwater level measurement of 4821 feet or higher, as specified below in Item 3c.
c. Baseline Data -The applicant has committed to using a baseline groundwater elevation
of 4818 feet, and a "trigger" of 3 feet. Therefore, upon a measurement of 4821 feet, the applicant
will need to begin implementing mitigation measures, ie: perimeter drains. The applicant's
commitment to collect monthly groundwater level measurements prior to installation of the
sluny wall will help to define the natural fluctuation of groundwater in the sun•ounding area.
If not already specified, the applicant will need to commit to notifying the Division within 48
