Failure to collect one annual water quality sample for GEM-2 -See explanation presented <br />for item 4 above. Basin requests that this point of the NOV be vacated. <br />6. Failure to collect one of two semi-annual field measurements for GEM-2 -See explanation <br />presented for item 4 above. Field measurements presented in the 2001 hydrology report for <br />GEM-2 during the May 2001 sampling was the data reported for GEM-1. A revised data <br />table for GEM-2 is attached for replacement in the report. Basin requests that this point of <br />the NOV be vacated. <br />In the future, Basin wil l complete the following steps to ensure that all samples are collected and the <br />appropriate analyses are conducted in the lab: <br />1. Basin will utilize the tables in the Permit as a checklist when sampling is conducted and in <br />completing the Chain of Custody identifying the analytical list that is sent to the lab. <br />2. Basin will ensure that Williams modify the shaft opening at GEM-2 to allow bailer access <br />for sample collection. <br />3. Basin will send all sampling results to Greystone as they are completed for a QA/QC check <br />to ensure all necessary samples have been collected. <br />4. Basin will include Greystone on the distribution list of results from the lab to complete a <br />QAlQC check to ensure all necessary analyses have been completed. <br />5. Basin will submit a summary of sampling to the Division on a quarterly bases to show that <br />all analyses are being completed as required.
