_ _ <br />Phase'IVb'Weekly Sninmary No. 33" ' " " Page 3 <br />., ..: _. 003-2203 <br />100-mil smooth LLDPE geomembrane over an azea of approximately 19,260 square feet in the ADR <br />liner expansion azea during this reporting period. Geomembrane panels X-26 through X-51 were <br />deployed, seamed, and tested by Poly-Flex. Destructive fusion and extension seam samples DFx-03 <br />through DFx-04 and DXx-03 through DXx-04 were mazked and removed. The results of testing on <br />destructive fusion and extension samples collected during this reporting period passed per project <br />specification. <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction materials <br />and soil materials placed were verified as suitable, and construction methods were in accordance with <br />project specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA <br />and the ADR Liner Expansion area for subgrade preparation, thickness, compaction, moisture <br />content, and materials. The Golder CQA Earthworks monitor performed three nuclear moisture- <br />density tests (SLNb-266, SLNb-267, and SLNx-04) and performed one nuclear moisture-density <br />retest on SLF that required reworking (SLNx-04a) located within the ADR Liner Expansion area <br />(designated by suffix `x'). The SLF met the requirements of the specifications prior to deployment <br />of geomembrane. Field test results are presented in Attachment B: -. -- .. <br />Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA, <br />verifying construction and layout per the project plans. <br />Golder visually observed placement and grading of DCF in the Phase IVb OSA. Golder CQA <br />monitors (one monitor per dozer when dozers are greater than 50 $ apart) observed DCF placement <br />and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations, <br />5 feet for 773 equipment, and 10 feet for haul road construction and equipment. <br />Golder visually observed removal of DCF from the Phase IVb VLF limits neaz Substation Hill. <br />Golder marked damage caused to the geomembrane for subsequent repair. <br />C=older observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder observed geomembrane deployment and repairs, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in the Phase IVb OSA met project specifications. Golder also <br />observed the repair and testing of damaged 80-mil single-sided textured geomembrane located near <br />the anchor trench by the Schmueser pipeline construction area on Substation Hill. <br />en~~w~s~~~n`a~~ee~~,~o03-~., GolderASSOCiateslnC. <br />
