Box 2905 <br />Durango, Co. 81302 <br />This was a complete random sample inspection. Those present <br />during the inspection were: <br />Harry Ranney, MLRD <br />Don May, NKC engineer <br />Jim Delazio, NKC foreman <br />Stephen Rathbun, OSM <br />Weather; Sunny, cool. Ground conditions were dry. <br />RECORDS REVIEW <br />Certificate of insurance: ?~ liability insurance policy is in <br />place with $500,000 limits, It expires on May 6, 1993. The <br />policy needs the "notification" statement revised. Mr. Ranney <br />indicated that this is being addressed by MLRD and that the <br />policy would be revised in the near future. <br />Bond: A corporate surety in the amount of $100,000 payable to <br />both MLRD and OSM is in place. A total of 350 acres are <br />bonded. <br />NPDES permit: P-860-423-406 expires 6-30-97. Two approved <br />outfalls exist. The second quarter, 1992 report was reviewed <br />which indicated no discharges. <br />Water monitoring: Ground water monitoring is occurring in <br />accordance with approved permit. Second quarter, 1992 <br />conducted on 6-1-92. There are no surfaC'e~water monitoring <br />requirements in the MLRD permit. <br />Air quality monitoring: Emission permit No. 82LP034F(2), which <br />was issued 5-24-82 allows 250,000 tons/year through crusher. <br />NKC is in compliance at this time. <br />Pond inspections: These are being conducted with the last <br />inspection being on 9-1-92. No problems noted. <br />Certifications: <br />Impoundments - Both sediment ponds are being <br />reconstructed(spillways) Upon com~let.ion both structures will <br />need to be re-certified. <br />
