<br />follows; <br />Diversions-- 1, 2, 3, 6, 10, 11, 13, 14, 15, 17 and <br />18. <br />Ditches--All road ditches with the exception of the <br />ditches on the road from the guard station to the mine <br />office. Although there are ditches along this road <br />they were also intended to be diversions which were <br />much smaller than the design indicated. <br />Culverts--4, 5, 7 and the third culvert from the <br />bottom of the access road to the north intake. <br />All of the above areas identified to be in noncompliance have <br />been addressed by DMG with a NOV. <br />Impoundments: Sedimentation ponds #3 and #5 are leaking. The <br />leak at pond #3 was discovered during the inspection. The <br />leak at pond #5 h~o existed since 1989 and the pond was <br />removed from service. The leak occurred prior to 1989 and <br />repairs were attempted withcut success. No further attempts <br />has been made to repair the pond. DMG has issued a NOV <br />addressing the main*_enance of both ponds. <br />Sedimentation pond #4 does not have any visible leaks however <br />it appears that water may be going out through the bottom of <br />the pond. This observation was made because the amount of <br />water entering the pond does not substantially increase the <br />amount of water contained within the pond. DMG should insure <br />that the company determines the reason for this and make any <br />necessary repairs or modifications to correct this problem. <br />Small Area Exemptions: SAE numbers 2, 4, 6 and 7 were not <br />maintained in a manner as to protect water quality. The area <br />within these SAES had bare ground and evidence of erosion <br />occurring. Straw bales have been used on some of the SAES to <br />treat the runoff from the area. DMG has issued a NOV which <br />requires demonstrations to be made for all areas indicated in <br />the mine play, to be a SAE and for failure to maintain a SAE. <br />Surface Water: There is no automated equipment at the surface <br />water monitoring sites. <br />Ground Water: Monitoring wells 5 and 6 were checked during <br />the inspection and were found to be properly protected from <br />surface water or contaminants from entering the wells. <br />9 <br />
