<br />CERTIFICATE OF INSURANCE: Current insurance is provided by <br />Federal Insurance Company through RHH of Washington, Inc.. <br />The insured is shown as Basin Resources, Inc. of Weston, <br />Colorado. The policy number is 37101173 and has an effective <br />date of 09/01/92 and an expiration date of 09/01/93. The <br />general aggregate amount of coverage is $2,000,000.00. The <br />coverage is provided for both the Golden Eagle Mine and the <br />New Elk Mine. <br />OWNERSHIP AND CONTROL: The officers and directors listed in <br />the mine plan for Wyoming Fuels Company have not been in <br />control of the mine since it was bought by Entech, Inc. in <br />June of 1991. The new company controlling the mine goes by <br />the name of Basin Resources, Inc. and is a wholly owned <br />subsidiary of Entech, Inc. which in turn is owned by The <br />Montana Power Company. An application for a permit transfer <br />was submitted to DMG in May of 1992. As of the date of this <br />inspection that submittal had not been approved, however DMG <br />notified OSM i*_ was approved on the Monday following this <br />inspection. The officers of Basin Resources, Inc. who have <br />owned and operated the Golden Eagle Mine since June of 1991 <br />have done so without a permit. DMG has not required <br />compliance with their approved program. OSM requested a copy <br />of the approval allowing the transfer of the permit but it has <br />not been received. Therefore a Ten-Day Notice is being issued <br />for failure tc obtain approval prior to transfer, assignment <br />or sale of permit rights. <br />BOND: Surety bond number 5700586 issued by Safeco Insurance <br />Company cf America, in the amount of S1,056,578.20, has been <br />assigned to permit number C-81-013. Wyoming Fuel Company, <br />10250 Highway i2, Weston, Colorado 81091 is listed as the <br />principal. A rider to bond number 5700586 in the amount of <br />5143,422.00 +:as issued effective 08/17/92. The principal <br />listed on the rider is Basin Resources, Inc.. <br />Since the bonding documents provided at <br />that Wyoming Fuels Company was the prin <br />of the bond a call was placed to Safeco <br />America in Seattle, Washington.
