<br /> <br />Apex <br /> <br /> <br />March 9, 1992 <br />Department and the Division. The quarterly pond inspections were conducted but <br />were not written up. The seriousness of the violation was downgraded to <br />insignificant. This lowered the total penalty to $250.00. The Division did not have a <br />copy of the insurance certificate so Mc Canning FAXed the Division a copy. <br />After the conference, upon reviewing the insurance certificate, Dan Mathews was <br />concerned with deficiencies with the certificate. The certificate of insurance did not <br />have the mine name, it did not list Sunland Mining Co. as the insured, nor did it <br />have a reference to any mine in Colorado other than listing the certificate holder as <br />'The Colorado Department of Mines and Minerals." Mr. Canning was contacted <br />March 9, 1992 and he said that he would have the insurance company change the <br />certificate. <br />2 <br />
