Current certificate of insurance. <br />2. 3rd and 4th quarter pond certifications <br />3. 4th quarter NPDES records <br />Abatement Date: February 24, 1992 <br />Termination: March 4, 1992 <br />Penalty Assessed: $450.00 <br />Proceedings: <br />Randy Price explained the reasons why the Division issued the notice of violation. <br />During the February 26, 1992 inspection, the missing information was noted to be <br />in the designated public file location. The NOV was terminated March 4, 1992. <br />The seriousness of the penalty was discussed. Mr. Canning stated that the insurance <br />had not lapsed; this was substantiated in the certificate of insurance.
