Banta, <br />Division Director <br />Please correct the MEIR supplement forms to reflect that alleged violation <br />5 of 5, combination spillways, was not in existence during the last State <br />complete inspection. The last State inspection was conducted on August 26 <br />through 30, 1991. <br />As discussed in the AFO March 29, 1991 and MLRD May 15, 1991 correspondence <br />regarding the combination spillway issue, operators were granted a grace <br />period until September 1, 1991 to have installed separate spillways. Copies <br />of each correspondence are attached. <br />Since September 1, 1991 was the completion deadline, there could not have been <br />a violation prior to that date. The last complete State inspection was <br />concluded prior to September 1, 1991. Therefore, the cited violation did not <br />exist during the last State complete inspection.
