11 <br />! 1 <br />LJ <br />n <br />LJ <br />June 24, 2003 -41- <br />6.1.4 Defect and Repair Observations <br />003-2203.7 <br />Golder inspected geomembrane panels for damage that may have occurred during manufacture, <br />transport, deployment, or installation as discussed in Section 6.1. Any damage noted {during or afrer <br />deployment) by Golder was reported to Poly-Flex and repaired. Golder repeatedly observed all <br />panels and seams for defects until Ames covered the geomembrane with either LVSCF or DCF. <br />Golder documented defects by assigning a unique defect identification code to each defect, and <br />marking each defect code on the geomembrane. The summaries of defects, repairs, and <br />non-destructive testing of repairs, including the approximate locations of each repair are presented in <br />Appendix IC-5 and L-5 for the lower and upper geomembrane layers, respectively. <br />6.1.5 Destructive Testine <br />Golder selected destructive test locations based on Poly-Flex's completion of approximately <br />69,619 lineal feet of welding for the lower geomembrane, 59,916 lineal feet of welding for the upper <br />geomembrane, various working conditions, and on seaming observations (e.g., suspicion of a <br />defective weld). Poly-Flex obtained and tested 163 fusion seam destructive samples and 30 extrusion <br />seam destructive samples during the installation of the lower geomembrane, and Poly-Flex obtained <br />and tested 129 fusion seam destructive samples and 9 extrusion seam destructive samples during the <br />installation of the upper geomembrane in the Phase IVb PSSA. <br />Poly-Flex constructed approximately 64,606 lineal feet of fusion welded seams and 5,013 lineal feet <br />of extrusion welded seams on the lower PSSA geomembrane, and approximately 59,720 lineal feet of <br />fusion welded seams and 196 lineal feet of extrusion welded seams for the upper PSSA <br />geomembrane. The specified frequency of destructive testing was one test per 500 lineal feet of weld. <br />Golder identified and Poly-Flex obtained destructive samples from the lower PSSA geomembrane <br />panel seams at a frequency of one destructive test sample per 396 lineal feet of fusion welded seams, <br />and one per 167 lineal feet of extrusion welded seams. Golder identified and Poly-Flex obtained <br />destructive samples from the upper PSSA geomembrane panel seams at a frequency of one <br />destructive test sample per 462 lineal feet of fusion welded seams, and one per 22 lineal feet of <br />extrusion welded seams.
