Phase IVb Weekly Summary No. ]3 Page 2 003-2203
Ames continued placement of Structural Fill (SF} in the Phase IVb area. Ames placed SF, hauled from
the SLF reject pile, to the Substation Hill south and west slope. Ames also continued slope
recontouring and re-compaction in the Phase IVb area on Substation Hill.
Ames continued drilling perforation holes into the 26-inch High Volume Solution Collection System
(HV SCS) piping at the staging area near the old LOB location.
Ames continued Drain Cover Fill (DCF) removal near the Phase III / Phase IV b PSSA east end berm in
preparation of end berm construction.
Poly-Flex began deployment of 100-mil smooth (upper) LLDPE geomembrane in the Phase IVb PSSA.
Poly-Flex deployed 100-mil smooth LLDPE geomembrane over an area of approximately 76,151
square feet in the Phase IVb PSSA during this reporting period. Geomembrane panels U-Ol through U-
19 were deployed, seamed, and tested by Poly-Flex. Destructive fusion seam samples DFu-Ol through
DFu-06, were mazked and removed.
On Thursday, April 17, 2003, Poly-Flex cleared, cleaned, repaired and tested two aeeas of previously
deployed (2002 construction) 100-mil single sided textured LLDPE and 100-mil double sided textured
geomembrane that was damaged by rock fall and equipment. Golder identified and inspected damage
panels and Soil Liner Fill prior to repair. Each repair was tested by Polyflex and documented.
Phase IVc
Ames resumed limited cut to fill activities within the northern limits of the Phase IV c area. SF and
oversize stockpiles were created for future removal or haul out.
Ames constructed a geogrid and Select Structural Fill (SSF) cap over UG-279. Ames placed two layers
of geogrid in accordance with the drawings and specifications. Approximately 2779 sauaze feet of
geogrid was placed in the working.
Field Monitorine and Testine Activities
Golder observed construction activities associated with Phase IV earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications. Removal of oversized and deleterious material from all fill
areas was observed. Golder observed Structural Fill (SF) placement in Phase N for materials, lift
thickness, moisture content, and compactive effort.
Golder observed placement of SLF in Phase IVb for subgrade preparation, thickness, compaction,
moisture content, and materials. Golder conducted one informational density and moisture content
test to verify SLF placed complied with the specifications.
Golder observed and documented remediation activities associated with the Phase IV underground
working construction. Workings were designated as Shallow or Deep as defined in the drawings.
PhmelVbli Aprll-APr19, 203 doc Golder Associates inc,
