Phase 1Vb Weekh Summary No. 28 Paee 4 003-2203 <br />Golder visualh~ observed removal of DCF from the Phase IVb / Phase I /Phase II tie-in area. at the <br />old LOB pad access haul road. Golder marked damage caused to the seomembrane for subsequent <br />repair during Phase IVb geomembrane tie-in construction. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2` wide by 3` deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. Golder also provided part-time observation for anchor trench backfilling. verifying <br />construction per project plans. <br />Golder observed eeomembrarte deployment and repairs. documenting ambient air temperature. panel <br />dimensions. thickness. seaming, tie-in overlap, and seam testing (trial seams. pressure. vacuum. and <br />destructive). Geomembrane placed in the Phase IVb OSA met project specifications. <br />Laboratory Tests <br />Golder obtained QA samples of Low Volume Solution Collection Fill (LVSCFb-14). Drain Cover <br />Fill (DCFb-I3 and DCFb-14}, and Soil Liner Fill (SLFb-20}. Laboratory index testing was <br />conducted on samples of Drain Cover Fill (DCFb-li and DCFb-14). and Soil Liner Fill (SLFb-20) <br />to verify suitability of materials used. Laboratory test results are presented in Attachment B. <br />Meetines <br />CC&V held Weekly Contractors' meetings on July 29 and August 5, 2003. Items discussed <br />included Safety. Environmental. Schedule. and CQ.A.
