On November 24, 1989 and January 19, Pebruary 20, March 27, April <br />17 and May 17, 1990, the Division sent letters reminding the o rotors of <br />their obligation to submit the DMR. <br />6. The letters sent to the operators by the Division were returned <br />indicating no forwarding address. The phone numbers provided 'n the rationale <br />to the permit were determined to have been disconnected. <br />7.
