COG-500045 <br />WELD COUNTY, COLORADO <br />TO: Ernest R. and Wanda E. Dakolioe <br />FINDINGS OF FACT <br />Pursuant to the authority vested in the Division of Adminiatrat <br />Colorado Department of Health ("the Division") by 25-8-301 to 3 <br />(1982), which authority has been delegated to me by the Executi <br />the Department, i hereby make the following Findings of Fact: <br />1. Ernest A. and Wanda E. Dakolioe ("the operators") <br />gravel pit, called Dakolioe Pit #1 ("the pit") located along W <br />Road 13 at the St. Vrain Creek crossing in Weld County, Colors <br />n of the <br />C.R.S. <br />Director of <br />rate a <br />Ccunty <br />2. In conjunction with the pit, the operators applied for <br />certification to allow discharges from the pit. Discharges are authorized in <br />accordance with Colorado Discharge Permit System Permit No. COG 500045 ("the <br />permit"), which became effective on October 1, 1987 and ie ache uled to expire <br />on September 30, 1992. <br />3. Ae a condition of the permit, the operators are re fired to <br />perform certain monitoring of discharge from the pit and to cub it a report <br />containing the results of that monitoring on a yearly basis ae pacified in <br />the permit.
