' RESPONSE: It is proposed that procedural deviations be recorded in the field notebook. BMR
believes that it would be overly cumbersome to try to note procedural deviations
on the chain-of-custody form. The chain-of-custody form is simply a form that
documents the person who is responsible for the samples at any point in time
from the time the sample bottles leave the laboratory until the samples are
' returned to the laboratory. It is not meant to be a field notebook to note other
procedures related to the sample collection and/or handling.
' 10. 7Tte preservation protocol for samples involves both acid (for »reralsJ mrd base
(for cyattideJ, and !ltis is always a source of potential error, since acidification
of samples for cyanide analysis will invalidate t/reir use. It would be a useful
error c)teck to have the laboratories receiving the samples for atrplysis run a pH
check on them as a check-in adjunct to assure no mix-up in pr~servntion even
before the analyses begin. Any such errors would dren be recorded immediately
' on the record sheets and correct samples requested.
RESPONSE: This is current standard operating procedure for the analytical laboratory (Accu-
' Labs) prior to conducting the BMR sample analyses (or any sample analysis).
11. Poge 8: Suggest that an additional copy of the chain-of-custody pnpenvork also
' be attached to -he outside of the cooler in a protective envelope tm guard against
destruction of this document if the inside plastic envelope Jenks and admits ice

' water.
RESPONSE: The chain-of-custody form is typically taped to the upper lid of th@ cooler and we
' have not experienced any problem with having chain-of-custody paperwork
BMR believes that this is unnecessary
destroyed
Therefore
.
,
.
' 12. Is adequate delivery in place to get the smrtples from San Luis to the lab sites as
required? How will this transport be done?
' RESPONSE: Yes, adequate delivery is in place, as all samples have been delivered to the
laboratory within the prescribed holding times. Tliis transport is done by ground
transport, using Halepaska and Associates, Inc. personnel tohand-deliver samples
' to Accu-Labs.
13. Page 9: What "corrective measures" cmt be token to bring smnple "wi[hin
' control limits"? ill i! impact the sample integrity (for exmnple, if the cyanide
sample were inadvertently acidified and then treated with caustic to give the
"correct" pH, the sample is no longer valid for cyanide analysac, evert tlroug)t
' "corrective measures" were taken to meet t/ie target pH in t/re sp»rpleJ?
' -18-

1
