OCT-iB-OT 10:53AN FR01FRl <br />,_ <br />Lr <br />+363 666 5385 <br />T-042 P <br />6 <br />the nearest blast hole to the nearest potentially affected structure either not located in the <br />permit area or not owned nor leased by the person who conducts the mining operations; <br />type of material blasted; sketches of the blast pattern including number of holes, burden, <br />spacing, and delay pattern (sketches will also show decking, if holes are decked to <br />achieve different delay times within a hole); diameter and depth of holes; types of <br />explosives used; total weight of explosives used per hole and maximum weight of <br />explosives used per 8-millisecond period; initiation system; type and length of stemming; <br />mats or other protections used; type of delay detonator and delay periods used; number of <br />persons in the blasting crew; and seismographic records, where required, including <br />instrumentation, instrumentation location and monitoring, name of person and firm taking <br />instrumentation readings, name of person and firm analyzing the seismographic record, <br />vibration and vibration level recorded. The Division further requires apre-blast survey of <br />potentially affected structures. Pre-blast surveys must be conducted at all structures <br />located within one-half mile of the proposed permit boundary unless persons in <br />ownership or control deny access to the structures. Written requests to conduct pre-blast <br />surveys must be sent to owners of all structuues a minimum of 30-days prior to initiation <br />of blasting. The Applicant must pmvide a plan for Division review and approval for the <br />conduct of the survey. The terms of the blasting condition aze set forth in the Division's <br />"Staff Recommendation to Approve." That condition is a part of the administrative <br />record in this matter and is explicitly incorporated herein by reference. <br />10. The weight of the evidence, together with the blasting stipulation described above, <br />demonstrates that the Applicant has met the minimum requirements of Mules 6.4,4(i) and <br />6.5(4). <br />Holcim/Coaldale, M-1977-247 <br />
