<br />'Phase IVb Weekly Summary No. 9 Page 2 003-2203 <br />On January 20a', a Caterpillar D9 wide pad dozer damaged the lower 100 mil single sided textured <br />geomembrane in the Phase IV PSSA neaz panel 354, creating puncture and scratch penetration <br />damage to the geomembrane. Ames used a Caterpillar 330 excavator on January 20a' and 21s` to <br />remove LVSCF from the area where the geomembrane damage occurred. As Polyflex was not <br />onsite, CC&V performed temporary geomembrane repairs on these areas on January 23`d, 2003. The <br />temporary repairs were surveyed and will not be covered with LVSCF until Polyflex returns to <br />perform certifiable repairs and tests on the damaged geomembrane area. <br />Ames performed SSF backfill on underground working UG-315, and performed SF backfill above the <br />previously placed geogrid at UG-316. <br />Phase IVc <br />Ames placed a Concrete Plug (CP) at UG-304. Ames placed Cemented Rockfill (CR) at UG-498, UG- <br />504 and UG-269. Ames placed geogrid at UG-504. Ames placed Structural Fill (SF) over previously <br />remediated workings at UG-234a, UG-236, UG-498, and UG-504. Ames performed excavations at <br />workings UG-304, UG-504, UG-269, and UG-274 in preparation of geogrid placement. <br />Ames performed local cut to fill of Structural Fill (SF) material down slope to the temporary stockpiles <br />above and below the old Connolly haul road elevation. <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. Golder observed Structural Fill (SF) placement in Phase iV for materials, lift <br />thickness, moisture content, and compactive effort. Golder obtained samples of SF for laboratory <br />index testing. Materials supplied met project specifications. <br />Golder observed placement of SF, geogrid, CP and CR for underground working remediation. These <br />materials were placed in accordance with the drawings and specifications into excavated or open <br />underground workings. CP and CR was observed full-time for correct aggregate size, QC observation, <br />cement ratio calculations, and placement and consolidation monitoring. <br />Golder provided full-time observation of LVSCF placement for correct material, road thickness, <br />placement thickness, drop height, taper, and liner damage. LVSCF was placed in accordance with <br />the specifications. Golder obtained samples of LVSCF for laboratory index testing. Materials <br />supplied met project specifications. <br />Laboratory Tests <br />Golder obtained four samples of LVSCF (LVSCFb-09, 10, 11, and 12) for laboratory testing. <br />Golder obtained one sample of Structural Fill in the Phase IVc area (SFc-OS) for laboratory index <br />testing.
