Phase IVb Weekly Summary No. 23 Page 3 00;-2205 <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks, All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed the placement of secondary and tertiary HVSCS piping! in the Phase IVb OSA, <br />verifying construction per the project plans. <br />Golder visually observed the placement of DCF in the Phase IVb OSA. Golder CQA monitors (one <br />monitor per dozer when dozers are greater than 50 ft apart) observed DCF placement and verified <br />minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations, 5 feet for <br />773 equipment, and 10 f et for haul road construction and equipment. <br />Golder visually observed removal of DCF from the Phase IVb PSSA east access road area, to allow <br />for tie-in to the upper Phase IVb geomembrane. Golder marked damage caused to the geomembrane <br />for subsequent repair. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA <br />for subgrade preparation, thickness, compaction, moisture content, and materials. The Golder CQA <br />Earthworks monitor performed five nuclear moisture-density tests (SLNb-164 through SLNb-168) <br />and performed 2 nuclear moisture-density retests on SLF that required reworking (SLNb-162a and <br />SLNb-164a) located within the Phase IVb .OSA. The retested SLF met the requirements of the <br />specifications. Field test results are presented in Attachment B. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in the Phase IVb OSA met the project specifications. <br />Laborato Tests <br />Golder obtained QA samples of Soil Liner Fill (SLFb-15 and SLFb-16). Laboratory index testing <br />was conducted on samples of Soil Liner Fill (SLFb-13 and SLFb-I~) to verify suitability of <br />materials used. Laboratory testing of SLF sample SLFb-i6 obtained during this reporting period is <br />pending. Laboratory test results are presented in Attachment B. <br />Meetings <br />CC&V held no Weekly Contractors meeting during this reporting period.
