OTHER PERMITS AND LICENSES REQUIRED EXHIBIT M <br />The following permits aze needed for the site: <br />1) County (special use) approval -this document is being supplied to the Montrose County <br />Planning office for approval. No new permit is needed since the azea of the permit is not changing. <br />2) A COE Permit is needed for the bridge installation near a water of the United States. This is <br />included in Appendix D. <br />3) An combined storm water -process water NPDES permit will be needed for the site since the <br />operation will dischazge groundwater from the pumping operation. This permit has been submitted. <br />4) A Gravel Well permit is needed from the Division of Water Resources since water will be <br />encountered in the extraction and evaporation will occur after reclamation. This decree is included <br />in Appendix D. <br />5) A fugitive emissions permit is needed for the extraction and processing operation. This permit <br />was obtained from the CDPHE. Any portable crusher/screen plant brought to the site will have its <br />own emissions permit. <br />6) A Spill and Containment Plan will be made by Western Gravel for the portable plants if any aze <br />brought to the site. No fuels, oils or grease will be stored on site since they are present at Western's <br />facility on the other side of the River. Any tank within the plants on site will have secondary <br />containment. <br />7) A Stormwater Management Plan is needed since the site will collect stormwater, mix it with site <br />water and dischazge it to the settling pond and then to the River. <br />North R-34 Pit 1/07 <br />38 <br />
