Phase 1Vb Weekly Summary No. 15 Page 3 003-2203 <br />Golder observed placement, compaction, and moisture conditioning of SLF in Phase IVb for <br />subgrade prepazation, thickness, compaction, moisture content, and earthworks materials used. <br />Golder CQA monitors performed sixteen nuclear moisture-density tests (SLNb-86 through SLNb- <br />102), three nucleaz moisture-density re-tests (SLNb-88a, 93a, and 96a), and twelve depth verification <br />checks (SLDb-61 through SLDb-72) on SLF placed within Phase IVb. The tests performed met the <br />requirements of the specifications and the nuclear moisture-density results summary are presented in <br />Attachment B. <br />Golder observed anchor trench excavation in the Phase IVb PSSA and OSA for proper location and <br />dimension (2' wide by 3' deep). Anchor. trench excavation met or exceeded the requirements of the <br />specifications. Golder observed anchor trench backfill in the Phase IVb PSSA for lift thickness and <br />proper method of compaction. <br />Golder observed construction of the Primary Underdrain in Phase IVb for trench dimension (5' wide <br />by 3' deep), slope (minimum 1%), materials, and geotextile overlap. <br />Golder observed construction of the Leak Detection Trench in Phase IVb for trench dimension (1' <br />wide by 1' deep), slope (minimum 1%), materials, geomembrane and pipe placement, and geotextile <br />overlap. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in the Phase IVb area met the project specifications. <br />Golder visually observed the placement of LVSCF in the Phase IVb PSSA area. Golder CQA <br />monitors (one monitor per dozer when dozers are greater than 50 ft apart) observed LVSCF <br />placement. <br />Golder monitored and observed the removal of DCF material in the Phase IVb /Phase III tie-in azea. <br />Golder CQA monitors verified width of lap required and marked the existing liner damage for future <br />tie-in activities and repairs. <br />Golder inspected the SLF finish grade under panels L-517, and L-436 on April 27, 2003. Golder <br />verified conditions were acceptable prior to repair and vacuum testing. Golder also inspected the <br />SLF finish grade under panel L-553 on May 2, 2003. Golder verified SLF conditions were <br />acceptable prior to deployment of the lower geomembrane in the azea prior to lower geomembrane <br />deployment and tie-in activities in the azea. <br />Laboratory Tests <br />Golder obtained QA samples of Select Structural Fill (SSFb-03), Low Volume Solution Collection Fill <br />(LVSCF-13), and Soil Liner Fill (SLFb-08 and SLFb-09) for index testing. QA laboratory testing <br />conducted and completed during this reporting period is presented in Attachment B. <br />Meetines <br />CC&V held a Weekly Contractors meeting on April 29.
