Long <br />Location: Section 12, T1N, R72W, 6th P.M., Boulder County°1PC1Or <br />Bond Amount: $43,277.00 as CD's <br />History: The co-permittees on this were issued three <br />violations on May 28, 1992, one of which was <br />for the spilling of tailings from the mill <br />building, tailings impoundment and pipes running <br />between the two structures. The co-permittees <br />were assessed civil penalties of $23,370.00. <br />8/14/94 Mill tailings were spilled on the ground from two places <br />at the mill building and from overtopping of the <br />tailings impoundment structure. <br />8/16/94 Division received several citizen complaints that <br />tailings had been spilled on the ground outside the mill <br />building and tailings impoundment. <br />The co-permittees were contacted concerning the <br />situation and scheduling of a Division inspection. <br />Wayne Tatman, COM, Inc. indicated only 100 gallons <br />were spilled and that clean-up was about done. <br />8/17/94 A.M. Division inspection - The Division met Wayne <br />Tatman, COM, Inc, and Max Yeager, contracted by COM, <br />Inc. on site. Mr. Tatman indicated that all tailings <br />were cleaned up. Tailings left containment at the <br />following locations: <br />(1) East Side of Mill Building - Through a 2' by 2' door <br />at ground level located below the thickener tank. <br />Tailings overflowed the thickener tank where they were <br />being stored, collected on the floor of the mill <br />building and eventually spilled out the door. Most <br />tailings were cleaned up. Tails spread from 25' to 80' <br />down hill from the mill building. <br />(2) North Side of Mill Building - Through a garage door <br />and off a concrete slab to deposit tailings on the <br />around 100' from the mill above the tailings <br />
