SUBMITTAL REQUIREMENTS FOR INERT FILL f (~
1. Written concurrence from the Colorado Department of Public Health and Environment for the use of a
Conditional Use Permit for the site to be flied.
2. Traffic impact analysis in accordance with Section 8-02 of the Development Regulations.
3. Purpose of fill, estimated life of the operation, and proposed after-use for the site.
4. Design and operation plan including the following:
a. Name, address and phone number of each responsible party;
b. Financial assurance;
c. Site description including the following;
1. Vicinity map;
2. Detailed map of site;
3. Legal description;
4. Geology; and
5. Hydrology including ground water level, gradient, quality, wells within 1,000 feet of the boundaries
of the fill, and surface water- ponds, streams, etc., on or within 1,000 feet of the site.
d. Fill material including description and source; and
e. Operations including the following;
1. Security;
2. Access, hours of operation, personnel, fees;
3. Location of fill on site;
4. Placement of fill;
5. Amount of fill including daily volumes and total volume;
6. Time span of operation;.
7. Inspection, control of fill material, and records;
8. Control plan to mitigate nuisance conditions including dust, litter, and noise;
9. Locations, quantities, and heights of any stockpiled materials;
10. Runon/runoff control;
] 1. Closure plan including the following;
a. notification;
b. final fill level;
c. cover materials including source, specification, and compaction;
d. revegetation and reclamation;
e. final grades and contours; and
f. notation of title;
12. Post-closure monitoring and maintenance including a responsible party and maintenance plan; and
13. MLRB Permit (if applicable) including number, special conditions and requirements, and expiration
date.
