This proposal is based on the assessment conference held <br />in our office on November 26, 1991. In attendance at that conference in <br />addition to you and me were Mr. Jim Hansen, Mr. David Berry, and Mr. Harry <br />Ranney. I have enclosed a copy of my Justification for Settlement Agreement <br />for your review. <br />On December 9, 1991, the Division Director, Mr. <br />penalties that I proposed for seriousness and f <br />penalty for seriousness from $250.00 to $750.00 <br />and impacts did occur off site. Fault was left <br />sets the civil penalty at $250.00 for fault and <br />total civil penalty of $1,000.00. <br />Dave Bucknam, reviewed the <br />cult.
