(The operator is reminded that all types of activities, such as pit roadways, topsoil <br />stockpiles, pitrun stockpiles and their drainage azeas, and equipment storage must be contained within the perntitted <br />area boundary J <br />Please provide better description of the seasonal placement and removal of fill for the river crossing, which may be <br />described in the Army Corps permit. <br />Exhibit E -Reclamation Plan (Rule 6.4.51 <br />The topsoil replacement depth is described as 6 inches to 1 foot, and elsewhere as a minimum of one foot. Please <br />clarify which minimum depth the plan commits to. This will be needed to calculate the bond amount. <br />There is no seed mix included in the revised exhibit. Please specify which species will be applied, the method of <br />application, the rate of application (of each species, expressed in "pounds of pure live seed per acre"), whether <br />mulch or fertilizer will be applied and at what rates. These details may be obtained as recommendations from the <br />USDA Natural Resources Conservation Service (NRCS). This will be needed to calculate the bond amount. <br />Exhibit F -Reclamation Map (Rule 6.4.61 <br />The map contains no labeled permit boundary line. Additionally, there is no delineation of the access road corridor, <br />though it is a part of the permitted azea/affected azea. Please revise the map to include this feature and the labeled <br />boundary line. <br />For post-reclamation features, the map only shows the pond outline. If a road is to remain on the site after <br />reclamation, please depict it also. <br />Please be reminded that the revised map should also include a signature and date. <br />Exhibit G -Water Information (Rule 6.4.7) <br />On the Exhibit C map there is a water well shown on the McGarry property, 200 feet from the edge of the proposed <br />pond. Depending on the type of well, please note that the Division of Water Resources (DWR) may require that <br />you obtain a waiver from that well owner, since it is located within 600 feet from a (proposed) pond. Since <br />minimizing disturbance to the hydrologic balance is within our jurisdiction, and ties is an acceptable method of <br />demonstrating such minimization, please ensure that DWR regulations aze complied with. If you have previously <br />made such inquiry, please provide a copy of the DWR response. <br />Exhibit I -Soil Information (Rule 6.4.91 <br />There aze soil descriptions included in this revised exhibit, but no soil map. Please provide a map showing the soils <br />on the site. Again, the NRCS is a good source for this. <br />Exhibit L -Reclamation Costs (Rule 6.4.12) <br />Given the lack of specificity in Exhibit E, as explained above, it is difficult to determine whether the reclamation <br />costs aze representative of the tasks to perform. Please provide greater detail of the individual cost breakdowns per <br />task, including type of equipment to be used, amount of material, distances, acreages, etc., as appropriate. Please <br />be reminded that during the above-water part of mining in each phase there will be a greater amount of acreage to <br />re-topsoil and revegetate, and the bond amount must include that. <br />Exhibit O -Proof of Filing Notices (Rule 6.4.17) <br />Please provide the signed and dated green cards of the certified mailings of the notices to the County <br />Commissioners and the Conservation District. <br />Exhibit S -Significant Man-made Structures (Rule 6.4.19) <br />
