The requested conference <br />Having peen Held as reyuirea by law, the Division and the operator hereby <br />ayree as follows: <br />1. Tne amount of the penalty shall ue 3 650.00 ; <br />2. The penalty shall be paid to the Division by the operator not later <br />titan 3V days from the dace Hereof; <br />3. Payment of such penalty by the operator shall constitute a waiver <br />of all further riyhts to review of such penalty or of the violation <br />on which it is based. <br />This agreement snall not be effective unless siyned by the operator within ten <br />oat's after the closing date of such conference, which date is set forth beloo-, <br />opposite the signature of the Conference Officer.
