Phase IVb Weekly Summary No. 5 Page 2 003-2203
November 9`~
Ames continued backfill of the interior anchor trench along the 9606 road on the Phase III ore slope
at the completion of geomembrane QA testing. Ames backfilled approximately 300 linear feet of
anchor trench.
Phase IVc
Ames performed excavation remediation activities for underground workings UG-307, UG-308,
UG-506, UG-507, UG-509, UG-550 through UG-555, and UG-557.
Ames performed confirmatory drilling activities associated with the Phase IVc construction area on
underground workings UG-304 and UG-310.
Ames placed Structural Fill (SF) over previously remediated workings at UG-270 and UG-505.
Ames placed Cemented Rockfill (CR) at UG-279 by tapering the working and placing 7 feet of CR in
accordance with IFC Drawing 12, Detail 1.
Field Monitorine and Testing Activities
Golder observed construction activities associated with Phase IV earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications. Removal of oversized and deleterious material from all fill
areas was observed. Golder observed Structural Fill (SF) placement in Phase IV for materials, lift
thickness, moisture content, and compactive effort.
Golder observed and documented excavation remediation activities associated with the Phase N
underground working construction. Workings were designated as "Shallow or Deep" as defined in
the drawings and will be remediated accordingly.
Golder observed placement of SF and CR for undera ound working remediation. These materials were
placed in accordance with the drawings and specifications into excavated or open underground
workings. Golder previously obtained samples of SF, Select Structural Fill (SSF), and CR for
laboratory testing. Materials supplied met specifications.
Golder observed backfill of the geomembrane anchor trench for proper dimensions, lift thickness,
compaction, and use of suitable material.
Golder observed geomembrane repairs, documenting ambient air temperature, seaming, tie-in
overlap, and seam testing (trial seams, pressure, vacuum, and destructive). Geomembrane placed in
Phase Nb met specifications.
Golder. observed and documented confirmation drilling activities associated with the Phase iV
construction. Full-time monitoring was given to confirmation drilling of underground workings,
PM1aze IVbi On2'I-Nov9.doc Golder Associates Inc.
