• (Page 3) • <br /> MINE ID R OR PROSPECTING ID 4 M-77-342 <br /> INSPECTION DATE 816198 INSPECTOR'S INITIALS ACS <br /> • <br /> this problem would be corrected by 8/7/98. <br /> A culvert installation at Station 87+00 was inspected. The culvert bedding for the single <br /> CMP being installed was being compacted at the time of the inspection. The same ephemeral <br /> drainage to be carried by this culvert flows out into the south access road alignment on <br /> the hillside leading down to the railroad. This drainage will be discussed in a subsequent <br /> section of this inspection report. <br /> The Con-9 conveyor crossing and the steep slope between Stations 74 and 86 were observed <br /> from a vantagepoint at the top of the steep slope. At the Con-9 conveyor crossing, a total <br /> of four bents are being installed to carry the conveyor over the perennial stream. An <br /> access road has been constructed to facilitate installation of the two bents on the north <br /> side of the stream. This access road comprises a small disturbance that is outside the <br /> area of affected lands defined in amendment AM-03. This disturbed area will have to be <br /> incorporated into the permit as discussed below. The Operator had notified the Division of <br /> this disturbance by a telephone call in May 1998. The Forest Service is requiring the <br /> Operator to stabilize the steep slope below Station 86 by plating with 4-inch durable rock. <br /> The installation of this rock is a change in the approved plan that must be incorporated <br /> into the reclamation permit as discussed below. <br /> The south access road was inspected. Installation of the permanent road configuration is <br /> scheduled to commence on 8/7/98. As discussed above; an ephemeral drainage passes through <br /> the access road location down the hill to the rail line. Flows in this drainage are <br /> related to snow melt and in response to storm events. The Operator will be installing <br /> itches and cross drains to isolate flows from the road. It was noted that topsoil <br /> salvaged during construction of the skid road for timber removal was stockpiled at the REC- <br /> 6 topsoil stockpile location (discussed below) instead of being stockpiled adjacent to the <br /> skid road as described in amendment AM-03. <br /> Installation of the mechanically stabilized embankment (MSE) associated with the conveyor <br /> snow shed was inspected. MSE wall installation was nearing completion at the time of this <br /> ins <br /> pection. Snow shed construction had not yet begun. <br /> he Operator has deviated significantly from the topsoil stockpile plan approved in <br /> mendment AM-03 . The operator had notified the Division of these deviations in May 1998. <br /> The primary topsoil stockpile locations in use were either observed or inspected. The <br /> largest stockpile is located across the highway from the left abutment of East Branch <br /> Reservoir Dam. Another topsoil stockpile is located along the access road on the west side <br /> of East Branch Reservoir. Topsoil salvaged from wetlands has been segregated from other <br /> plant growth medium in the stockpiles. Both wetland and regular topsoil are clearly marked <br /> and identified by the placement of signs. A third topsoil stockpile location at REC-6, <br /> along the rail corridor, was inspected. The rail embankment drops-off steeply behind the <br /> topsoil stockpile, which will make it difficult to grade the stockpile to 3 :1 as required <br /> by the plan approved in amendment AM-03 . The Operator must incorporate the revised topsoil <br /> stockpile locations into the permit, and must provide a topsoil stabilization plan specific <br /> to the AEC-6 stockpile, as discussed below. <br /> It was agreed that the Operator would submit a technical revision application to address <br /> the following issues related to the Henderson 2000 conveyor installation project: <br /> •1•' Revised topsoil stockpile locations and a specific topsoil stabilization <br /> plan for the REC-6 stockpile that will prevent loss of topsoil off the side of the rail <br /> embankment. The Operator is reminded that Rule 3 .1.9(4) of the Hard Rock/Metal Mining <br /> Rules specifies that "Once stockpiled, the topsoil shall be rehandled as little as possible <br />
