This form must be completed and submitted with all requests f6r-mirior'~
revislons, as defined in Rule 1.04(73), technical revlslons`a's~defined in.
Rule 1:04(136) and permit revislons, as defined in Rule 1.04090). .All
revislons are to address the requirements of Rule 2.08.4.= Thrr'e (3> copies of
your revision including maps must be submitted in order for~l~t to be complete.
All revisions are to .be formatted so they can be inserted ~in~o the permit .to
replace the revised sections-„maps, tables and/or figures. Explain in the
revision what section 1s being revised. The revision date 'srould•be printed
in the toxer right corner of the revision pages. If necessary, the Table of
Contents should also be .revised. When the final version of the revision is
approved, the Federal .operation 1s responsible for providing one copy to each
of the agencies noted on the Federal Agencies Mailing List: Ftlr non-Federal
mines, the operation is responsible for providing one copy to OSM in
Albuquerque (625 Silver Avenue, SW, Suite 310, Nex Mexico 87}02). The
revision may not proceed until OSM has been provided copies: Copies should be
sent certified mail.
