Permit Revision Adequacy <br />July 6, 2004 <br />Page 3 of I S <br />13. On page 2.04-41, 151 paragraph, "age" is more appropriately referred to as "residence time," <br />consistent with the Mayo report. <br />14. On page 2.04-42, the last full sentence on the page states that "the overall relationship between <br />the fault offset and the damage zone water storage and inflow rates becomes obvious." Please <br />describe that relationship for clarity and continuity. <br />15. On page 2.04-43 the first statement notes MCC's water handling systems. Please provide a <br />page or section reference to that discussion in the text. <br />2.04.7 Hydrology Description <br />16. A minor point on page 2.04-46, please indicate in the text that figures SA, SB, SC, SD, SE, and <br />SF aze contained in map pockets and not in the text. <br />17. The Division requests that MCC add a map that provides a reference for visualizing <br />groundwater flow in the azea of the permit, This would show the areas undermined and the <br />strike and dip of the strata across the area and at least allow for plotting of existing monitoring <br />well sites on the map. This map will assist in the evaluation of probable hydrologic <br />consequences, monitoring plan, and in the points-of-compliance evaluation (Rule 4.05.13.) <br />18. On page 2.04-48 the brief discussion on regional connectivity of stratigraphic units should be <br />explained more fully, describing the units that crop out up-dip (and thus have limited recharge <br />potential from the "active zone"), those that crop out down-dip within the permit area and the <br />observations of groundwater discharge from those down-dip outcrops. Please develop this <br />discussion more fully. <br />19. Please correct a minor typographical error on page 2.04-51 -first paragraph, 5`h line <br />("aquifer2"). <br />20. On page 2.04-51, in the penultimate paragraph, note that the well completion and <br />abandonment data discussed have not been incorporated into Exhibit 12, to date but reside in <br />Volume 11A of the PAP. Please correct. <br />21. On page 2.04-52, 2nd paragraph, the statement is made that new wells have been added, <br />including Upper and Lower Dry Fork and SOM-3B and that the location and completion <br />information can be found in Table 5. Table 5 (page 2.04-58) is a summary of slug test data, not <br />location and completion data. Please correct this reference. Further, please include slug test <br />data for those new wells as available and note on the table the unit in which each well is <br />completed. <br />22. While complete analyses of groundwater quality aze contained within the annual hydrology <br />reports, as noted on page 2.04-53, the Division requests that MCC include a discussion and <br />analysis of the data within the permit application package (PAP) in response to Rule <br />
