Mr. Wallace Erickson <br />May 4, 1995 <br />Page 2 <br />Because the permit boundary was staked from a 1" = 400' scale map <br />the symbol on the map itself is approximately 30' wide. Since there <br />has been minimal disturbance in this area, i would like to discuss <br />this matter with the Division further before any additional <br />disturbance or remedial action takes place. <br />Violation #3 pertains to existing haulroad certifications contained <br />within the permit application. OSM stated that the 'as-built' <br />certifications for Haul Road "A" and the Mine Entrance Haul Road do <br />not reflect the actual conditions observed during the inspection. <br />In particular culvert F-3, which discharges into sedimentation pond <br />006, was not on any of the haulroad certification exhibits. This <br />culvert is shown on Exhibit 13-2, Surface Hydrology Map, which is <br />a certified map. A review of the 'as-built' exhibits for the <br />haulroad certifications for both Haul Road "A" and the Mine <br />Entrance Road will be initiated. A minor revision will be submitted <br />to the Division for the exhibit that contains culvert F-3, and any <br />other exhibits for which conditions have been modified since the <br />original certifications.
