A cover sheet to the application correctly identifies <br />the permit as C-81-025. <br />*Rule 2.08.4(5)(b) A specific description of the requested change in the terms of the permit; <br />The application is in compliance with this rule. Revised pages to the permit provide a description <br />of the requested changes in the terms of the permit. <br />*Rule 2.08.4(5)(c) A specific description of any changes in the mining and reclamation operation <br />which mny hreve an effect an performance bond requirements of Rule 3; and <br />The application is in compliance with this rule. There is no effect on the performance bond <br />requirements. The cover sheet of the application indicates no change. <br />*Rule 2.08.4(5)(d) Such other information as mny be necessary jot the Division to determine if the <br />proposed revision will cotrtply wish the Act and these Rules. The Board or Division mny reasonably <br />request additional information to evaluate the proposed revision. <br />The application is in compliance with this rule. Information in the application is complete. <br />Rule 2.08.4(6)(c)(ii) Within 10 days after the filing of a complete application for a minor <br />revision, the Division shall issue a decision approving or denying the application in whole or in <br />part. A written copy of the decision shall be promptly provided to the permittee and shall be <br />posted in the Division office. <br />A proposed decision date of March 27, 2001 was posted on March 26, 2001. A written copy of <br />the proposed decision was sent to the permittee by Division letter of March 27, 2001. <br />Rule 2.08.4(6)(c)(iii) Within ]0 days after the notification required by 2.08.4(5)(c)(ii) above, any <br />person which has an interest which is or may be adversely affected may request a formal hearing <br />on the decision by the Division. Such request shall be in writing and state with reasonable <br />specificity the reasons for the request and objections to the Division. If so requested, the formal <br />hearing shall be conducted in accordance with the requirements of 2.07.4(3). <br />The public notice of Rule 2.08.4(6)(c)(ii) above was posted until April 9, 2001. There were no <br />comments in response to the public notice. <br />Rule 4.15.5(2) /n no case shall grazing be allowed to preclude or interfere with postntining <br />vegetation sampling ns required in 4.15.8. <br />Grazing had been permitted on the reclaimed areas.
