110(4) 3.4 Notice of Intent to Continue <br /> Except as provided in Pule 1 .6, the operator shall file on the <br /> anniversary date of the permit a notice of intent to continue mining <br /> operations. The notice of intent shall include an annual fee of <br /> fifty dollars and a map or statement that shall indicate: <br /> (1 ) The land affected during the precedinn year, <br /> (2) The land to be affected during the cominq year, and <br /> (3) Any land reclaimed during the preceding year. <br /> - 28 <br />
